Progesterone is the hormone released after ovulation that prepares the uterine lining for a possible pregnancy. Because it rises and falls sharply across the menstrual cycle, the timing of the blood draw matters more than for almost any other hormone.

What progesterone measures

After an egg is released, the follicle it came from becomes the corpus luteum, a temporary structure whose main job is producing progesterone. That progesterone thickens and stabilizes the uterine lining so a fertilized egg can implant.

This is why progesterone functions as confirmation that ovulation occurred. Before ovulation, levels sit near zero. Roughly seven days after ovulation, they peak. If no pregnancy occurs, the corpus luteum breaks down, progesterone falls, and a period follows.

A draw taken around day 21 of a 28-day cycle targets that peak. Drawn at the wrong point in the cycle, a low result means very little.

What high or low levels mean

Low progesterone during the expected luteal peak suggests ovulation may not have occurred, or that the corpus luteum is not producing adequately. It is relevant to irregular cycles, difficulty conceiving, and early pregnancy loss. Low levels are also expected and normal after menopause.

High progesterone is most commonly explained by pregnancy. Outside of pregnancy it can occur with ovarian cysts or, less often, adrenal conditions.

Because a single measurement captures one moment in a moving cycle, clinicians usually interpret progesterone alongside cycle timing and symptoms rather than in isolation.

When in my cycle should this be drawn?

About seven days after ovulation, which is roughly day 21 in a 28-day cycle. If your cycles are longer, shorter, or irregular, the right day shifts, so confirm timing with your clinician before the draw.

Does a low result mean I did not ovulate?

Not necessarily. The most common explanation is a draw taken outside the luteal window. A low result at the correct time is more meaningful and generally warrants a repeat test to confirm.

Does hormonal birth control affect this test?

Yes. Most hormonal contraceptives suppress ovulation, which keeps natural progesterone low and makes the result difficult to interpret against standard ranges.
